When dealing with patients, it is important to remember that patient health information needs to be protected by healthcare providers. This involves the secure transfer of information from the patient to the healthcare provider, as well as future protection from unauthorized usage of PHI without the patient's consent.
This act established a national standard for electrical healthcare transactions and also for providers, health plans, and employers.

Because healthcare providers are legally liable for any actions they take, it is important that they ensure they gather medical records properly in the event that legal action is taken against them for their performance. Here are a list of the things a Medical Record should have.
- Patients Medical History
- Patients ID, DOB, Full Name, Medical Record Number, SSN, Full Address and Phone Number, Marital Status.
- Dates of appointments as well as any treatments given, admission times, discharge times, diagnostic tests run and ordered.
- Results of tests and consults
- Information given to patient regarding treatment in regards to symptoms and ailments
- Physical examinations and records of results, including instructions given to patients for their treatment.
- Medications and prescriptions with dosages
- Informed consent and other legally binding forms that allow practitioners to practice medicine on patients.
This leads to an important concept in medical care, but every patient has to consent to procedures done. This could be implied consent but is often informed consent. Informed consent comes from informing the patient of the procedure and asking them if it is okay to perform the exam. This requires the patient to be informed of the procedure, the risks, the benefits, the alternative treatments, etc...

PPE or Personal Protective Equipment is a broad term used for any equipment used in any work environment that helps protect the wearer of the gear. This could include things like surgical mask, gloves, gowns, eye protection, hair nets, hand washing, etc...
Hand washing is essential from procedure to procedure.
